City of Thousand Oaks has a $75,000 bond on file. We will be accepting customer claim form through September 30, 2022 to submit the first claim against the bond. Customer Claim Forms received after September 30, 2022 will be submitted to the bond for reimbursement if there are any funds leftover.

 UPDATE 7/1/2023:

Per TOMC Section  5-22.07, the City will no longer be accepting new claims after six (6) months from the date of the health club closure. This does not  affect the rights of consumers to alternatively pursue civil remedies which may otherwise be available under State or Federal law.

The City have verified claims received and the insurance company has agreed to a total. There will be an agenda item going to City Council on July 18, 2023 to approve the claim.

Next Steps: If approved, the insurance company will send the City a check in the mail. The City will then mail (or email) a release form to members which needs to be returned prior to refund issuance. Once the release form is received by City, the City will mail refund check to member within 30 days.
