Form 700 - City Council Statement of Economic Interests
The City Council Members are elected officers identified in Government Code Section 87200 and file statements of economic interests with the City Clerk Department.

Copies of the statements of economic interests filed by the City Council Members (Al Adam, Claudia Bill-de la Peña, Andrew Fox, Rob McCoy and Joel Price ) may be obtained by visiting the offices of the Fair Political Practices Commission (FPPC) or the City of Thousand Oaks - City Clerk. The physical address of the FPPC is 428 J Street, Suite 620, Sacramento, California 95814. The physical address of the City of Thousand Oaks - City Clerk Department is 2100 E. Thousand Oaks Boulevard, Thousand Oaks, CA 91362.

The statements of economic interests for some state and local government agency elected officers may be available in electronic format on the FPPC's website.


Form 806 - Public Official Appointments
Fair Political Practices Commission (FPPC) requires each agency to post a single Form 806 (FPPC Regulation 18705.5 amended 03.2012).   Form 806 reports additional compensation that officials receive when appointing themselves to positions on other committees, boards or commissions of a public agency, special district, and joint power agency/authorities.  Each agency must post a single Form 806 listing all paid appointed positions including the date appointed, length of term and stipend amount.
